to everyone, thanks for showing your support. it's quite sobering to know that my case is not an isolated one. my husband and i decided to finally mail my AOS application yesterday even if the i-693 form has not been signed by the civil surgeon. i also sent in the vaccination record provided by the clinic and the one from st. luke's. we were thinking that we would just deal with an RFE when and if it comes. our primary concern now is to be able to continue on with the process so we could rest for a while from all this "immigration-related" stress. lolz.

I agree. I almost had a fight with my hubby about the CS thing.

He keeps saying that all CS clinics he called wants me to do the whole medical and I keep telling him is that all they need is to transcribe my DS3025.

So, to avoid more aggravation, we used Dr. Arnold's services.

Unfortunately, USCIS lost all of my medical files that I took at St. Luke's.

So, I still ended having a new medical done despite that I had been thru it.

It is crappy because they're ripping people off.

I feel bad for those who have no idea with the visa processing and stuff.

They probably did the whole medical without visiting VJ. Hehehe

The answer to the question on the DS-3025 is in the AOS sticky thread: FAQs: Forms I-693 and DS-3025, Medical and Vaccination Supplements When Adjusting Status

Q: Why are K non-immigrant visa holders asked to include a vaccination supplement when filing Form I-485 to adjust status?

Some countries allow K non-immigrant visa applicants to defer their vaccinations until after they arrive in the U.S. If the beneficiary's vaccinations were not completed as part of the visa process, it is necessary to include form I-693- part 1, part 2-vaccination section, and part 5 in the application.

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

Determining if you had all your shots is the hard part for most.

I vote for not unnecessarily repeating the medical; but please, consider carefully getting the recommended immunizations if you haven't already. They're for your protection and protection and everyone else. I have permanent damage from what is now a vaccine preventable disease. It's too bad the vaccine wasn't available when I could have benefited from it. There are good civil surgeons out there. We found one with one phone call - even cheaper than Dr. Arnold.
