If you have to complete it - according to your vaccination records from St. Lukes - then you must take the shot before you file for AOS. A civil surgeon will transcribe all your records to I-693a form that will go along with your AOS paperwork.

To prevent an RFE it would be best to get the shot before having the I-693 transcribed. Many VJers use Dr. L.E. Arnold in Texas to transcribe the DS-3025 through the mail.

The phone is 512-444-3131 if you cannot find a local civil surgeon for a reasonable fee.

Naturalization

3/23/14 - N400 package sent to Phoenix

3/27/14 - N400 package delivered

4/3/14 - NOA1 receipt date

04/29/14 - biometrics date

07/01/14 - interview date

xx/xx/xx - Oath Ceremony

My answer is yes but still will depends on the civil surgeon. Because the civil surgeon that i had here in NY still asked me to complete the 2nd dose of the vaccine before she can transcribe and signed my I-693. Which is alright for me because I really needed to have the shots but its just so pricey. Goodluck to u.

hi there!

In my case,St.Luke's gave me HPV shot #1 then when I went to civil surgeon,he told me to have HPV shot #2,it cost $150 for the shot if I will have it on his office because USCIS doesn't take insurance for immigration purpose,so he told me to go to our doctor or other clinic to have the shot so we won't need to pay $150 and so that's what we did.We went to the clinic close to his office to have the HPV shot #2 using my husband's insurance. Then civil surgeon, transcribed my vaccination form from St.Luke's to I-693 including the HPV #2,signed and sealed it and he charged me $25 and we filed my AOS the next day. That's it.

Then after 2 months when I had the HPV #2,I had the last shot which is HPV #3.Because it was written on the record that I need to have the last shot after 2months which is the time frame and since it's appropriate on my age,why not have the last shot so I had the completed HPV vaccines, and it's good also to avoid cervical cancer.

So I suggest, have the HPV so you won't get RFE.

Good luck!

Thanks for the replies so far. Dylan had the first dose at St. Lukes....then the next at an OB -GYN in the Philippines. She is not due for another dose until Aug. 3rd...just trying to avoid problems etc. It appears there is a civil surgeon here in our city. We just want to file the AOS etc..and not have to wait until after Aug 3rd but it seems that is what we need to do. Thanks again for the replies...
