Packet 3, should I send any form along with the checklist doc?

I received packet 3 about 3weeks now and while Im gathering the documents needed I realize that I have to send the checklist first ( I knowIm so stuuupid) now my question is do I have to send the checklist paper only or theres I should send some other forms with it? also it says something about confirm,ation receipt?? what receipt??

help pls Im really confused ...

All I know is that most of the forms have been replaced with DS-160

Hi sorry I havent been around tehese days, and No actually I havent gotten any answers well I didnt quit understand and Im sooo confused...:s any help???

All you have to send back is the checklist as well as DS 230... Also, complete the DS 160 application online...( i sent a confirmation receipt of this application as well, donno if they needed it). Hope this helps.
