hi. i came here on a B2 visa last june and met my husband and got married by October. We are now filling all the papers we need. my question is: what name do i put on all the forms? my married name or my passport name?

I'd say your married name and include a certified copy of your marriage license.

Correct Married name which do not make the mistake I did. The format in the Philippines is first name, your maiden name is then the middle name and then the husbands name as the last name. You can also get a new passport which will show your name correctly along with other IDs like postal ID, drivers license, etc.
