So..I'm about to mail this off to the Doctor in Texas, I have two questions.

The receptionist told me to sign it ONLY, do not fill anything out on it....but on the actual file it says that the person requsting the medical and vaccines must fill it out....

Also...do I put my maiden name or my married name??

I'm pretty sure you only need to sign it. I didn't do anything else on mine.

Put your married name, and make sure you sign with your married name too!

17th May 08 - Got married!!!!! :)

18th June 08 - Mailed AOS/EAD/AP to Chicago lockbox

3rd October 08 - Green card in hand!!

26th August 10 - Sent I-751 to VSC

31st August 10 - NOA1 from VSC

10th January 11 - I-751 approved!

14th January 11 - 10-year green card in hand!!

22nd April 23 - N400 submitted online; NOA available in USCIS account immediately
6th November 23 - Interview; approval same day
28th November 23 - Oath ceremony scheduled

The physician is supposed to fill it out. They do. Only sign it and attach a memo with all the information they need to complete the form. They are very efficient.

NOA AOS, EAD, AP: 8-27-2008

Biometrics: 9-18-2008

AOS Transferred to CSC: 9-25-2008

Requested EAD Expedite: 11-12-2008

EAD Card production ordered: 11-12-2008 changed to 11/17/2008 Why? (I hope it doesn't change every week!)

Received AP: 11/17/2008

Received EAD: 11/22/08 (Praise God!!)

AOS RFE: 1/29/2009

AOS Approved: 3/24/2009

Called USCIS 4/1/2009 told no status change and case not yet reviewed from RFE request.

Received green card: 4/3/2009

Laura....I just spoke with Dr Arnold's office. The receptionist told me the reason for having all the information on a separate sheet is because USCIS is picky about how it is put on the document. So rather than have it sent back due to a misplaced "," or "-", the dr office will complete the document for us.

I'm sending mine off today toooo.

April 13, 2009 - Welcome to the USA letter rec'd. PRC to be rec'd within 3 weeks.

April 16, 2009 - 2 yr GC rec'd in mail.

March 2, 2011 - Mailed in I-751 to CSC

March 4, 2011 - I-751 delivered to CSC

March 4, 2011 - NOA issued

March 12, 2011 - NOA received

April 14, 2011 - BIO Appt 5/6/11

May 6, 2011 - BIO done in 10 minutes...no re-takes this time :)

June 27, 2011 - Email rec'd "Status - Approved"

July 1, 2011 - 10 yr GC arrived
