I am sorry to bring this up again but I am still confused. My wife is taking my last name, so should we fill out the forms with her maiden last name or with her new last name? The confusion comes in when on some of the forms there is no spot for maiden name or other names used, so how will they know? For example on the I-864, under principle imigrant, do I put her maiden name or married name?

If she is going to use your last name now that she's married, then you should put her new married name everywhere. When asked for her maiden name, put her maiden name. Your marriage certificate will show the reason for the name change.

There is a spot on the Biographical Form (325) for maiden name, no? Also, they do ask for it at the Biometrics Appt - all other names used.
