An I-94 is for any Non-US citizen entering the U.S. from abroad. It is the Arrival/Departure card.

What is the status of your fiancee? Married, or not married yet?

Does she have AP (Advance Parole)?

Is she still within the 90 day window to get married?

Do you intend to marry? If not, then she needs nothing other than a return ticket. If she intends to return to the U.S.A then she either needs to have AP to leave or she will need to have the K1 (assuming you're not married yet) reissued in her country.

An I-94 is arrival/departure card they stapled into her passport. She does not need to fill out another to leave.
