some DS156 question

Finally, im all done with the ds156 and ds157, I have attached 1 page to my ds157. the numbers 12 and 17 questions , i answered it in different page because the space wasnt enough in the ds157 form. I just put (see attachment please)

did u fill up ques.# 8 and 9?

Yes, in ds156

8."other surnames used" i put my father's surname (thats what i used when i was still single)

9. "other "first and middle names used" i put my first name and the middle name i used when i was still single (means my mother's maiden name)

the number 8 and 9 , simply mean your maiden name ( surname , (8), and full complete name and middle name used (9).
