hello to all this is a problems of my closed friend who is trouble about the status of her passport,she is a widow before she get married for her second husband,but she used in her passport is the family name of her first husband which is died already and about also her second husband she is annulled already but as usual even if she is married again for the second husband she did not change her family name used still using the family name of the first husband,her question is can she allow to change her passport using her maiden name?because she dont want any huzzle if she will start the process of her fiance visa in the coming days,if its okay to change her into her maiden name,can u guys tell me about the requirements for amendments of passport?i am in cebu and would to amend it in here in cebu.hope u can give me all the information about the requirements to bring to DFA for the amendments...thanks in advance waiting for all your replies.god bless u all.

yes she can change her name back to her maiden name. just bring certified true copy and photocopies of her annulment decision and finality and the annotated marriage certificate from LCR and NSO.

Yes, she can change the name back to her maddin name, just take a orgianl brith cir. her husband death cir. and s.s. card if she is from the u.s.a. and prove of address. U should not have a problem
