On form I-765 question 11 it asks, Have you ever applied for emplyment authorization from USCIS? Which USCIS office? Date(s)? The question I have is what do I put for date?

I mailed it on 07/21/2006
USCIS received on 07/24/2006
Card was approved on 10/12/2007

Check the dates that you listed on your post, I think the wrong year was posted. I would use the date that USCIS officially received the application. This is the date they have a record of.
Toria and Salvador
