Ok, thanks Hank.  I just thought I might have to show proof and back up the information I put on the form so they know the amount is correct.  I guess I don't need that.  I already have my W-2 and latest tax return from 2017 printed out.  I just need to print and sign the I-134 form now I guess...

The only monetary info you need on the I-134 is your GROSS annual income.   Assets and such are necessary.

Ok, so that is just #3.  My annual income is:   $#####  part right?   I just wasn't sure if I needed information to back that up, but I guess that is what the W-2 and tax information is for.  So since I don't need assets then I should be done with everything.  Thanks again.

It's ok thanks.  One last question.  I don't need a statement from my employer like it says in the below section do I?  The W-2 and taxes suffice enough for that right?  Thanks again.

If you are not a new employee then you should not need an employer letter either.
