New Zealand Police Clearance Authorization Form

Hi, I hope I'm on the right place to ask this. I haven't looking all over the internet but I can't seem to find the right information. I am trying to request for Police Clearance from New Zealand and I can'f find what values to put in the form specially for the Applicant Case Number and A#.

 

I hope someone can help me.

Are you using the form from here?

 

https://travel.state.gov/content/dam/visas/Supplemental/ACK - Auckland.pdf

You will only need Applicant case number or A#. Applicant case number is your NVC case number

Requesting Office: Consulate General of the United States

Requesting Office Email Address :AucklandIV@state.gov

Applicant case number : NVC case Number

 

All applicant requests must include:

• A copy of the completed N.Z. clearance request, which must include: The Post case number  (e.g. ACK for Auckland) or  A#
• A photocopy of the applicant’s passport biographic page
• The NVC BAR CODE SHEET  if the clearance is to be returned to the NVC.

I appreciate this, but just when I was about to fill this form, I was advised that this no longer is the right form to use. NZ has changed their policy so instead of having to request through the US consulate, individuals will be the one to request directly to the police vetting department.

 

I truly appreciate your help, it clears everything up.
