Need help with a Question about form i130, 46.a-50

I am filing i130 form for my mom. She has been in the US 5 years ago through visitor visa and stayed for a year . 

In the form asks: if the beneficiary is currently in the US complete the following, which it asks for visa type and i94 number. Do I have to fill that part or just put N/A?

 

Also on the passport number, should I put her current passpot ? The one she visited before has been lost.

For I-130 (version dates 02/27/17) clearly states the following: 

 

If the beneficiary is currently in the United States, complete items 46.a. to 46.d.

 

So, the question is: is your mom in "currently" in the United States? If the answer is No, then I believe that N/A is appropriate. If she is currently in the United States, then you need to answer the questions with the appropriate information.

 

As to the passport number. You should enter her most current unexpired passport number that she currently holds. 

 

Be sure to mark question 45 as "YES" since your mom was in the United States previously.
