Beneficiary's Employer 1, street number and name are missing.
Beneficiary's occupation currently says 'ACCOUNTING', should it say 'Accountant' instead?
Line 54. Maybe "in May" instead of "on May".
Part 3, Multiple Waver Request checkbox missing, (5.d., if not applicable)
