error when making new account for setting interview

I tried setting up an account for my wife for the interview but all i get is error says to double check spelling of name and etc. But i typed everything correct. Im worried whats going on?

Few things to check:

 

No special characters, no commas or periods.

 

 

Visa chase and citizenship.. DONE!

Another:    First name and middle name are given name, last name is surname.

 

Without seeing what you have it is almost impossible to figure out the error.

Still error. 

It just asks for 

Email

First name

Last name

Password 

Confirm password 

Write the letters from box

Well your screen shot only shows the titles for the sections, and your phone charge is still good.   Do you have a computer?

 

Last suggestion, only use the first name, no middle name, and last name just as asked.    Did it confirm the password for both entries are the same?

The person whos helping me with the visa process said i should just call the embassy.

Ive done everything on computer and mobile and with different email adress.

Do remember, embassy is closed Monday, Philippine holiday.
