Name as it appears on Permanent Resident Card

Hi all - I'm filling out the N-400 application for naturalization. For the name section, it says you should print "your name exactly as it appears on your Permanent Resident Card".

 

At the top of the card my name is "Last name, First name and middle initial" - but at the bottom, in the code section, it has my full name "Last name<<First name<Middle name".

 

Do I go with the middle initial, or the full name? Thanks!
