DS-3025 has nothing checked in the summary

It has to be a USCIS assigned doctor not just anybody.

Hahaha....That is the funny thing it was a doctor from their website.

It is all good. I got a hold of another doc and they accepted our DS-3025 form. The appointment is tomorrow

We are trying to determine if we need to do the I-693. We have our DS-3025 sheet and it has 4 vaccines given and dated. However, in the summary it has neither the complete box checked or nothing in the US vaccination requirements not complete.

Do we need to have the I-693 completed by a certified surgeon?

I don't know of any that have the completed box checked and sent a copy of the DS-3025 with their I-485 .... but I highly suggest attaching a copy of page 4 of the I-485 Instructions with SECTION 7C highlighted! There are adjudicators at USCIS that don't know the requirements for the K-1 and will RFE an application because of their ignorance. Include the copy of page 4!

I personally contacted USCIS on this topic and have an email reply confirming only the vaccination record is needed except for those with special medical conditions

May I follow up with the OP about this? How did it turn out for you? My DS 3025 doesn't have any box checked as well on the Summary and St. Luke's also gave me three shots last year with dates. We filed our AOS last February and we took a risk on the RFE. So far, we haven't received an RFE and we are now scheduled to have our interview on September 12. I am now worried that they will give us an RFE on our interview date because of the incomplete DS-3025.

Current timeline:

 

July 2019 - filed I-751

August 2020 - filed N400

Sept. 2020 - i-751 status "Case Was Transferred to a New Office". Transferred to NBC.

April 12, 2021 - N400 interview in LA passed (with a pending i-751)

May 20, 2021 - I-751 status changed to Approved and N400 status changed to Oath Will Be Scheduled. 🤩

May 21, 2021 - received a call and email from USCIS Los Angeles with my NOA with my oath schedule.

May 28, 2021 - oath taking schedule in USCIS DTLA office. 😍

Most who have gone through the AOS using their DS-3025 don't have a problem. I suspect you won't as those who have had problems with medical know it before they land in the states and take the necessary steps to complete once they arrive.  Stop worrying. You're going to be fine.

 

Once you're in the states it is VERY VERY rare that you don't pass your AOS and Permanent GC.  As long as you can prove that the two of you are a couple with an eye toward co-mingling your household then there are no worry.  The hardest part of this is getting your visa. Now most of it is submit the paperwork and wait for your approval.

 

By the way. The OP hasn't been on VJ since December 20, 2016 so I doubt they will answer your questions.

Did you attach a copy of page 4 of the I-485 instructions with section 7c highlighted?  The issue is with adjudicators that don't know the USCIS process correctly.

 

I emailed USCIS on this a few years ago, there should not be an issue for the majority of cases.  Only those with special medical conditions were required to have the I-693.    No the USCIS Liaison did not list what "conditions"

Not many would know to include the page 4 of the instructions to circumvent problems unless they have read your pass posts.
