DOB wrong on DS-3025

My husband and I recently married and are filing for the AOS so I'm triple checking all forms to make sure we have everything and have copies of everything to mail this out tomorrow. Everything looks good except for the DS-3025 medical I had done in London in August. They have my date of birth wrong by 1 day and I have no idea how the DR managed to do this. It's clearly an error on their part so I'm wondering what I do regarding this.

Do I need to call someone or can I just add in a little side note to say this in incorrect information?

Also slightly unrelated to errors... Do I need to include my chest x-ray from the medical with the AOS package?

*** Thread is moved from the K-1 Process forum to the AOS/Family-Based main forum -- OP is at the AOS stage. ***

Include a note if you want. They might not even notice the error.

The chest x-ray is yours to keep, toss, or wallpaper your bathroom with.
