Vaccination and Medical Report

Hello to all, my my first time here and I have a couple of question please. Maybe someone can explain to me about the process.

My fiance have interview this coming Wednesday for K1 visa. She took her medical exam and vaccination few days ago covering all shots including flu shot. She was given a CD, receipt and sheet of yellow paper just show the shots she took signed by the doctor (not DS-3025). They told her the results will be sent to the Embassy. Now my question is this. Is she suppose to get a copy of DS-3025? Also when we do the AOS here, does she still need to see approved doctor and fill out I-693? Please if someone can explain to me the process for AOS I-485 MEDICAL PART only.

Yes, you should have been given a copy of the DS-3025, but a few countries do not provide one.

If the paper with the list of vaccinations received shows that all the required ones were given at the time of the K-1 medical, then you can skip the civil surgeon unless instructed to see one via an RFE or the IO at an interview, should you have one. Stick a note on the cover letter and add in a statement about having all the required vaccinations, and that the DS-3025 should be included with the rest of the medical documents that were turned in at your port of entry and forwarded to the USCIS. Place a copy of the signed paper you were given at the medical with that statement.

Thank you so much. The doctor gave her immunization or vaccination yellow book/record listing all the shots, dates and signed. They told her to keep it with her all the time...so if we skip the civil surgeon do we still need to fill submit and fill I-693? Thank you!

Thank you for sharing the link with me. I read it and its a big help now...appreciate it! We will just complete the I-485 and make a note with it about the vaccination and medical as well. If there is going to be RFE then will see...hopefully not if USCIS will have the medical file with all records if they don't lose it, but hopefully they will not!
