My fiancee already has one notarized with both her parents signature. She just needs to fill out that part but idk wat to put on the last line. Her cfo appointment is tomorrow and there is nothing on google about this. Edited June 29, 2016 by Sheenadao

My fiancee needs help filling out the advise uppon intended marriage part. Her parents already signed it and got it notarized and her cfo appointment is tomorrow. Any help or ideas would be greatly appreciated. Here is the link to the form she has

Thats what i was thinking. Thanks for your help. I thought it was weird that i had to put my name twice so i was getting confused.
