adding extra information to a form when you have run out of room

Form G-325A

My fiancee has had six addresses in the past five years.

there is only room for five.

Is there a specific method or format I need to use to add the last address on a separate sheet of paper?

And while I'm asking questions:

The last fields on the form says to "print" the applicant's name.

Should I not type that in? Should she hand write that since it says "print" ?

You use a separate sheet of paper, indicate the question in which you are providing more information on it, add the info, sign and at the bottom. On the form make sure to write "see attachment" in that question box. My husband also had to do this for addresses.

Print simply means do not use cursive or "write"
