St. Lukes Extension Clinic (SLEC) Medical Examination Experience

I had my Medical exam Last Tuesday, 15th of December at St. Lukes Medical Center (Extension Clinic) at 1177 J. Bacobo Street, Ermita, Manila. I decided to have my medical examination three (3) weeks before my interview despite that Im having my tonsillitis and taking my antibiotic plus theres a typhoon in Manila called Nona. I flew all the way from Davao City on the 14th after I had my check-up with my physician just to confirm if I wont have a problem with the medical due to my tonsillitis (just making sure about circumstances that might happen). I stayed in Cherry Blossom Hotel, its located same street with SLEC about 2-3 blocks away, beside Robinsons Place Mall.

To start with, these are the things that you need to bring on your medical exam. (NOTE: These are for the Immigrant applicants)

1. Passport

2. Photocopy of passport biographic page (1copy only)

3. Online Registration form (if you registered online. 1 copy only). For those who havent registered online, you will be given and fill up forms in SLEC.

4. Interview Letter (2 copies)

5. 4pcs. 2x2 pictures (no earrings, ears must be visible, white background, with collar) write your full name at back of every picture.

6. Ballpen

My first day (Dec. 15, 2015)

Step 1: I went to SLEC at 3:00am, lucky I was number 2 in the line. Then at 4:00am, the guard started checking documents, gave us priority numbers, stamped the back of our hand, then we were allowed to go inside the clinic.

Step 2: I was sitting inside waiting to be called since the process will start at 6:00am. I was number 2 and asked to go directly to counter J. I gave my requirements to the lady and she asked me few questions like who is my petitioner, do I have other relatives petitioned with, my date of birth, and some other basic questions. After that, she asked me to scan my right pointing finger 4x. and took me a picture. Then asked me to proceed at the cashier in 5th floor.

Step 3: At the cashier theres a guard that will guide you in the line. I paid an exact amount of Php 11,300.00 (NOTE: they accept credit cards and checks and dollars). Please remember to have your receipt and passport with you always; they will keep on asking for these in every window. After paying I was then asked to proceed to get my vital signs, height, weight, get my eyesight checked on the same floor (5th floor). Then, I was told to go directly to 3rd floor to have my chest x-ray and 4th floor for my laboratories (blood test and immunization interview) before I go back to the 5th floor for my Physical Examination.

Step 4: Went to 3rd floor for my x-ray (NOTE: remember your first and last day of the most recent menstrual period), everything seems good then directly proceeds to 4th floor to get my blood tested, it was fine. Then, I had my immunization interview with a physician in the 2nd floor. I was asked with some questions like; do I have an immunization record with me, did I have chicken pox before, have I traveled outside the country for the last 12 months and some basic questions. Then she told me that Ill be receiving 1 vaccine only the next day and that I am not allowed to have sexual contact for a month (LOL! Sounds funny. Husband is in the US).

Step 5: Went back to 5th floor for my Physical Examination, I was guided by personnel in a room where a physician is waiting. She then asked my some questions like; about pregnancy and abortion, drugs, previous hospitalization and operation if any, major illnesses like cancer, aids, tuberculosis etc. and few basic questions. Then the physician asked me to remove my clothes (Yes! Everything including undergarments). She checked my skin, breasts, teeth, palpated my abdomen and assessed my #######.

Step 6: After everything, I was told to proceed to the 1st floor at the check out counter. I was given a paper (a piece of things I have to bring on the second day) and was given instructions. Told me to come back the next day at 7:15am for the result and immunization. Then I went back to my hotel at exactly 9:00am. Wow! That was fast!

My Second Day ( Dec. 16, 2015)

Step 1: I arrived at St. lukes at around 7am. Showed my receipt and passport to the guard and asked me to directly proceed to the 2nd floor for the immunization. I was given a shot of MMR and thats it Im done. I am then given instruction to go to the 1st floor level for my result.

step 2: In the 1st floor, after few minutes of waiting I was called to proceed straight to window 8. Results were given to me with some questions (basic questions) and instructions (CD and immunization record to be shown on the port of entry while sealed envelope must brought with me in the interview) (NOTE: Do not open he sealed envelope). And that was it, I am totally done smoothly without hassles at exactly 9:00am.

You are very much welcome. I just want to help those who will have same situation with me in the future. I was diagnosed with bilateral tonsillitis a day before my medical and was on antibiotic medication and was too worried that time, that it may affect my medical examination. I was scanning and searching the internet if there was someone who has same situation with me but haven't found one. So I made this for future reference of the others who will undergo the medical. Everything went well on my medical exam. Thank God.

I see that you filed for a F-2A...was also wondering if the required immunizations are the same for a K-1 (if you happen to know?)

No I dont have a copy if my immunization record but Im pretty sure I am not a fully immunized child as my mom told me because she always forgot to bring me in the center for immunization. And yes I only got 2 vaccine injected on me and thats MMR. They didnt give me vaccine for varicella cause I already had a chickenpox when I was young.

And about the K1, I think yes because there are K1 applicants who happens to have their medical on the same day I had mine. One I talked with, she gets MMR and varicella vaccine.

No I dont have a copy if my immunization record but Im pretty sure I am not a fully immunized child as my mom told me because she always forgot to bring me in the center for immunization. And yes I only got 2 vaccine injected on me and thats MMR. They didnt give me vaccine for varicella cause I already had a chickenpox when I was young.

And about the K1, I think yes because there are K1 applicants who happens to have their medical on the same day I had mine. One I talked with, she gets MMR and varicella vaccine.

OK..thank you so much for the fast response and info! That's good news :) My fiancée was worried how many immunizations she would need to have at SLEC. She told me she was fairly certain she received all immunizations when she was a child..but...her Mom doesn't have her immunization record. Her Mom is trying to obtain a new record from the hospital/clinic/midwife my fiancée always went to as a child (if that's possible), but we don't have an answer yet. So it's good to know that even if she doesn't have her immunization records, maybe only 2-3 shots maximum is all she will need at SLEC. Isip ko sobra masaya siya sigurado...salamat po!

OK..thank you so much for the fast response and info! That's good news :) My fiancée was worried how many immunizations she would need to have at SLEC. She told me she was fairly certain she received all immunizations when she was a child..but...her Mom doesn't have her immunization record. Her Mom is trying to obtain a new record from the hospital/clinic/midwife my fiancée always went to as a child (if that's possible), but we don't have an answer yet. So it's good to know that even if she doesn't have her immunization records, maybe only 2-3 shots maximum is all she will need at SLEC. Isip ko sobra masaya siya sigurado...salamat po!

Timeline

Do I still need to bring my Baby book (vaccines and boosters list when i was a baby). Is it still important? I lost mine.. if yes, i think i will be going back to my pedia and hopefully my records are still there..
