Hello everyone,

I emailed the US Embassy here in Jamaica asking for them to email me back with my medical interview appointment letter (Packet 4). However they just emailed me back all the stuff I got in packet 3. There is a 1 page form in packet 3 which basically just allows for me to declare if i had/have any medical conditions. That is not an appointment letter....??? What is the ACTUAL appoint letter, shouldnt it have my scheduled interview date on it? Only thing I can think of is my confirmation page for my interview, is that the appointment letter??? Also the form for the medical, its the I-693 correct? Do I print out that myself and take it to the medical?

Call Andrews and make your appt for your medical. They will accept the confirmation page. I don't believe interview appt letters are sent out anymore. There is a link to the medical exam form available. We printed it out and my fiancé had it all completed and took it to the appt with him. if not they have them there to complete. It's not long or complicated.

you have to create an account here to pay

https://ais.usvisa-info.com/en-jm/iv/signup/iv

after paying you will get the option to select from available dates, which are only on a Friday.

No, those forms were replaced by the DS-160 (thats what I read in my research). Prepare your documents and you are set to go.

I must mention that if you have not done your medical as yet you will need the Medical History form that the embassy sends to you in packet 3 to take to your medical. So you should still pursue packet 3 from the embassy.

You're welcome.

Good luck!

This is great information. I thought that the visa was paid on the day of the interview and that the Embassy made the appointment for the interview. if we jump ahead of packet 3 and 4 which has the medical paperwork and checklist don't we run the risk of not getting medical done in time for interview that we set up when we do the DS-160? I am a bit confused. Thanks in advance for your help.

Thanks interview set and payment thanks so much do we still need to do a ds156 or ds156k form

I will be in Jamaica the date of your interview, I would so love if that was my fiance's interview day too, but may be too soon for us to get that day. Blessings.

You do need the Medical History form you get in packet 3 to complete and take with you to your medical. It is a very simple form, just one page.

....and yes, if you 'jump ahead' of packet 3 and 4 and schedule an interview too early, you do run the chance of not getting your medical and other required documents done in time. It can be done though if you estimate your timing reasonably, its just your 'risk' to take.

good luck!
