My wife is the sponsor and the U.S citizen, we got married on a k1 visa and she is going by my last name now with the social security office and DMV. On the affidavit of support should she put her name as maiden or married?

She put her married name, but her pay stubs say her maiden name.

If she changed her name through marriage, then she will be using her married name from now on. She can use her married name on the AOS forms, even if her maiden name shows on her tax return and pay stubs. You will be including a copy of the marriage certificate. They can see how her name changed.
