Hello,

Can someone confirm that form I-693 is not needed when applying for AOS? I am submitting the vaccination report since I don't have a completed I-693 and the medical exam was taken in Dominican Republic in May.

I believe that you need to include a completed 593 to prove you have had all the vaccines. We had to submit one with one shot that was missing transcribed to it for my wife's shot profile to be complete. One shot of a 2 part series she could not get in in time for her flight. Let others chime in.

I'm interested to hear other's experiences as well. Waiting for our interview next month, we completed I-693 but didn't keep a copy (d'oh), should that be a problem? Still have almost a month, maybe we should request a copy from the medical office?

On form I-693 the firs section is personal data, the rest is filled out by the doctor. I just read the instructions that say that the I-693 is not needed unless the medical exam was conducted over a year ago. And that the vaccination record is required as part of the evidence for the I-485.

An I-693 is not needed when adjusting status from a K-1. You only need a copy of your vaccination report form DS-3025, as long as it is complete and shows you have all the required vaccinations.

We only submitted the DS-3025 and our AOS was approved.

K-1

I-693 is not required for k-1 and you can just submit copy of ds3025. However, if your case takes a long time to process, or you get an officer that looks at your vaccination record, they could send you an RFE for I-693.

We sent our ds3025, waited and waited, then got an rfe for expired medical, so we did a new medical and transcribed all the vaccines at that time. We sent it off in June and are still waiting.
