hello all,Is the form 1040 (tax return) just a copy from the actual 1040 form that you fill in your taxes,or comes in a transcript summery format after you request it from IRS ?

You have two choices here: 1) a COMPLETE tax return copy with W-2s and all schedules, or 2) a tax transcript ordered from the IRS. The tax transcript is free and avoids the possibility of an RFE for an incomplete tax return, so a tax transcript is what is recommended here on VJ.

Depends what u ask for. If you ask just the transcrips, they will send so which shows required information in summarized form.

If u ask for the copy of tax return, u will get the copy of what u filed.

Most embassies ask for return transcript (the image you uploaded) while few ones ask for full return copies. Check embassy site in your country. I think the one you have is good enough.
