I-134, Q 9 and Q10

How does Q9 and Q10 different on the I-134 form? In the past I have submitted an affidavit of support for my ex-fiancee (no longer associated), and ex-wife.

If you filed an affidavit of support for anyone in the past, then list them in #9. If you have filed petitions for anyone in the past, then list them, along with the current fiance(e) you have petitioned, in #10.

for Q9. should I put my fiancee name cause its I submitted the only and first I-134 for the I-129 f and now its the second one for the interview,

for Q10. same issue, the only visa petition i have submitted to USCIs was the I-129 for my current fiancee.

Date Submitted: The day you submitted your I-29F.

No. If your current fiancee is the only one you have ever petitioned before and have never previously filed an affidavit of support for anyone, then #9 would be None. You list her in #10. See example > http://www.visajourney.com/examples/Form-I-134.pdf
