Typically you need most recent tax return or IRS transcript, and proof of income, such as letter from employer and/or recent pay stubs. The USC petitioner and co-sponsor each provide an I-134 and supporting documentation. Check over the instructions for your consulate first to see if they specify anything.

Do we need a statement from our banks?

Thanks,

If you are not using assets or non-taxable income to meet the requirement, then no need for bank statements.

If you do get a statement from the bank, who does the bank address the letter to? Is it to the consulate/embassy, or "to whom it may concern"?

Just the most recent year and proof of current income.

Probably makes no difference, but you might want to go with 'To Whom It May Concern' if you want to reuse the letter for AOS later.
