What "forms" do I need for IV & AOS package

HI,

I've read that it's better/faster (and cheaper) to mail the IV and AOS packages at the same time in a single package. However my cover sheet for the IV package says "Attn: CMR", while my cover sheet for the AOS package says "Attn: CMR AOS". The address is otherwise the same for the two.

I just want to know which "attn" I should tell DHL to address the package to. Or does the different "attn" field mean I definitely need to split them into their own separate packages?

On travel.state.gov it talks about sending "all of your forms" with the IV and AOS package.

I only have one "form" which is the I-864EZ. I see no form associated with the IV application. Am I missing something? What other "forms" are they talking about?

I never got a "document cover sheet" from the NVC, I just got the barcode page. I never got a welcome letter from the NVC. I have my case number and AOS and IV fees are already paid...Where do I get the document cover sheet?

The barcode page should be fine since all of the "cover sheets" contain the same bar code.

If you would really like a document cover sheet, you can try calling them and they can e-mail you one though it'll probably take a few days.

As long as the bar code page is sufficient, I'm fine. Online I click the link "Print document cover sheet" and it just gives me the barcode page, which is why I was wondering about the page with the checklist that I've seen. I guess they don't want that anymore?

I would call the NVC and ask for the welcome letter. The welcome letter checklist is to be attached to the top of the AOS/IV packet unless they have stopped doing that in 5 months since we sent in our packet.

Well, the document checklist is the one that is included with the welcome letter which, you never received.

But, again, it's not necessary since the barcodes are all the same and they'll just be scanned by a computer anyways.

I just called NVC and they said I don't need the document checklist, that the bar code pages are fine. But if I want it anyway, it would take 1 week to receive.

October 3, 2011: Emailed checklist response (checklist generated due to typo on Form DS-230)

November 10, 2011: Interview - APPROVED!!!

December 18, 2023:  Filed I-90 to renew Green Card

December 21, 2023:  Production of new Green Card ordered - will be seeing USCIS again every 10 years for renewal
