Incomplete copy of DS-3025

Hi there,

I have my medical information and my K-1 visa issued and in hand.

I'm pre-preparing my AOS stuff.. The civil doctor here when I had my K-1 medical gave me a copy of my DS-3025 which he had completed, but ONLY page 1.

Do i need BOTH pages for my AOS? I just want to make sure I don't get an RFE or rejection because USCIS may claim that my DS-3025 is not a complete document.
