you need I-963 form. Make sure to use the last version of the form which expires 03/31/2017.

There are some medical exams too besides vaccinations.

DS-3025. That's what I've read in the forums and that's what I sent my fiance to bring with him for his medical exam in Haiti. That way all his vaccinations can be accounted for which will make the medical portion of the Adjustment of Status portion less stressful.
