Sorry to ask something that I figure must have been asked a thousand times, but here goes. Do I need the I-693 if she had her vaccinations with her medical at St. Lukes? Do I need to schedule an appointment with the civil surgeon?

If you are sure that all vaccines have been given then send in the DS-3025 and attach a copy of the following:
