Adjustment of Status - Work Authorization I765

Timeline

I was hoping someone could help me. My wife worked in the US 2 years ago for about 20 days and her company had her fill out forms for authorization. The question on the I765 asks if she ever applied to USCIS before. She does not have the documents from 2 years ago.

Should we mark no for applying to USCIS or mark yes and attach supplemental information stating she does not have these documents?

Any help is greatly appreciated.

Was it a non-immigrant work visa that she received 2 years ago? A work visa is different to employment authorization from USCIS, which is what they are asking for on the I-765.
