You do not send an I-693 yourself. An I-693 must be filled out and signed by a civil surgeon, or do not send one at all.

If you check the link I posted, you will see the USCIS instructions for a K-1 AOS applicant. The medical is valid for a year. If you had all the required vaccinations at the time of the K-1 medical and they are marked off on a properly filled out DS-3025, then you can skip the civil surgeon if applying for AOS within a year of having the K-1 medical exam.

If you check the link I posted, you will see the USCIS instructions for a K-1 AOS applicant. The medical is valid for a year. If you had all the required vaccinations at the time of the K-1 medical and they are marked off on a properly filled out DS-3025, then you can skip the civil surgeon if applying for AOS within a year of having the K-1 medical exam.

I'm not sure if we have a copy of the DS-3025, that's why I'm a bit concerned....He hasn't arrived yet (in about two weeks! yay!) so I'm just trying to think of it all. Do you know what we should do in the even that we don't have a copy of DS-3025 on our hands?

I know for a fact he had all his required vaccinations done during the medical prior to the interview, so at least that much I know haha.

The original DS-3025 will be with the rest of his medical documents inside the sealed envelope he hands over at his POE. They will be sent to the USCIS.

You can write on the cover letter, and even include an extra statement to make sure they see it, that all the required vaccinations were completed and marked off on the DS-3025 at the K-1 medical and they can find the DS-3025 among the other medical documents in his file.

Kay, my husband had to have another medical. Does he list the medical on the cover letter as it's own separate Item? Meaning, the medical is not a part of any of the forms?
