I am about to make an appt to get my husbands vaccines ready for AOS. I have a quick question, he never got anything back about his medical from DR, he never got his vaccines, just the general exam- but we do not have any documentation of this..Does USCIS have those papers? what form do we need to bring in to the doctor?

DR could be mailed that to USCIS. ask dr. office. if no request it. it should be in sealed envelop and do not open it!

I just called the doctors office here in NYC to ask about the vaccination supplement for my husband since we are getting ready for AOS. He did not get his vaccines back in the dominican republic, and we have no records of previous vaccines. So the doctor in Manhattan said that it will cost 300 dollars to get all of the vaccines. Is this a normal price to get all of the vaccines and get them transcribe? I do not know what to do. Help!! Also, we did not receive a copy of the DS 3025, but i assume USCIS already has this in their files.

Most K1's get them done before they get here, from what I remember that sounds relatively cheap.

Hi Stefani,

If you want low cost vaccines go to Fort Green Health Center in Brooklyn. They are at a VERY low cost there. You should try to get a copy of your DS-3025 because it will let you know which vaccinations he needs. Once you get a copy of it, you can either fax or take the copy to Dr. Anthony D. Blau (212) 766-4433). He charged us $60 to fill out ONLY the supplement form after my husband got his vaccinations from the health center in Brooklyn.

You will likely have to call around and see if you can find a civil surgeon that will agree to do what you are proposing.

Yes, you can get them elsewhere and have a civil surgeon transcribe them. He will need the MMR, Tdap/TD, and varicella(unless he already had chickenpox). Flu shot only needed during flu season, which it is not right now.

Also, yes, the USCIS will have all the medical documents. They were included in the sealed envelope that was handed over at your POE.
