I129F question about previous wife ( previously petitioned k1)

Hello on the I-129F I have a couple questions,

Background, divorced from my American wife in 2003, met Filipina, petitioned her on a K1 and her daughter on a k2 in 2004, was approved and she came here we got married and life was good, fast forward, it went bad, we split up and filed divorce after 9 yrs marriage. ok so I met another Filipina, want to petition her, in filling out the I-129f question 11 and 12 ( prior spouse) on 11 I answered my American wife, on 12 I will have my ex Filipina, so it ask name, family,first and last, do I use the name on our divorce papers? which is her name for almost 10 yrs now, or do I use her maiden name?

then question 14 asks if I ever filed before, yes, so it asks name ,, ok so there I use her maiden name? or name wen she was married to me? it then asks filing date, I do not have that, I remember it was june of 2004, but don't know exact date, what do I do? and what if she refuses to give me her A number, the form asks that also. then 14h says result? I guess I write approved?

now also do I list her daughter in this also for the k2 at same time?

one more question I have, it asks my new fiancé mailing address,, she don't have one! she lives in the remote mountains of Cebu island, no street or givin address,, so how do I tackle that question?

q 36,, are there different consulate locations in Philippines? or is the answer simply manila?
