Confused about adjusting the status

Hi guys, you've been a big help when I was still under the process of k1., and now I'm here to ask for another help., I'm in the states now for almost 2 months and just got married last week. We're now on the process of adjusting my status, however were confused about the medical transcription (vaccine records from the Philippines) we don't know if I have to undergo another medical exams here in Vegas since my visa has anotation that says MED CLASS B. Do we have to submit the papers for adjustment first or we need to see a civil surgeon first? Thanks in advance

I was only given shots for MMR and I guess the boxes below the paper given to me by st.lukes are blank

You can send just the DS 3025. It should be good for 1 year. The I-693 is required if the DS 3025 has expired. Did you bring a copy of your vaccination records during your SLEC medical?

You mean send the da 3025 form from slec together with the other documents for adjusting the status? Yes I have the copy of vaccinations from slec and the disc

Yes. Just send your DS 3025 with other stuff for AOS. No need to get the I-693 since your DS 3025 should still be valid.

I was asking whether you brought your vaccinations from when you were a baby to the SLEC Medical exam. I did not have a record of my vaccinations from when I was a baby and I only got two shots from the SLEC medical exam. At the bottom left corner of my DS 3025 a box about incomplete vaccinations was checked. I sent only my DS 3025 and no I-693 needed.
