How do you do the "Parental Advice" letter that needs to be notarized? Do you have by any chance a sample for this?

I and my fiance met when I was 13 and he was 16 (same city here in the Philippines). That makes us 23 and 26 respectively when we got engaged :) If by any chance, how can you create this CFO letter? Samples? :)
