Questions about Proof of US Citizenship (Born Abroad)

Hello, I am a US citizen (born abroad) applying for the I-129F. Under question 13.d.1, it asks for the 'Certificate Number' from the 'Certificate of Citizenship.'

However, my original certificate of citizenship has no such number, I attached a copy of what it looks like. What should I do about this?

I am also planning to send a copy of my US passport, but am unsure what it means when they say "validity for 5 years." My passport expires in 2016, will this be a problem?

Thanks for the help!

What you have there is not a Certificate of Citizenship which is why it doesn't have the number in question (if you google the term you should get plenty of images of what the CofC looks like).

That is not a certificate of citizenship. It is a certificate of birth abroad. They are 2 completely separate things.

You can just use your passport as proof of US citizenship. The validity period of 5 years means from issue date to expiration date. For example, if your passport was issued in 2006 but expires in 2016, then your passport has a 10 year validity.
