Timeline

On the DS-260 the question states:

Have you ever been refused a U.S. Visa, been refused admission to the United States, or withdrawn your application for admission at the port of entry?

If the approved petition was returned to USCIS and then later withdrawn is that considered a refused Visa?

If it was the petition that was return and withdrawn that is not a visa . A visa was never issue in that case. They may or may not ask about that petition at the interview, it depends on how far along was the petition, But to answer the question on the DS-260 no that was not a visa
