AOS Questions...Clarifications

I am hoping to send my AOS today but better be sure...I tried browsing for same questions but hope you don't mind if one or two of my questions have been answered....

q1: for the G325A, do I only need to send 1 copy from me (applicant) and the USC spouse (sponsor)?

q2: on the I765, which category should I put? I came on a k1 and married within the 90 days requirement and sending in EAD and AP with the AOS...©(9) or (a)(6) ?

q3: does the I864 need to be notarized?

q4: I am taking risk in sending in the vaccination record without transcription of a CS, will it be a problem ?

q5: in filling out I485, do wee need to maintain what we filled out on the DS160 before? like for example other name used? or the maiden name if the "other name used" now since I am already married to my then fiancé?

also,if I wrote an organization , should I do the same?

lastly, on the question have u ever been arrested, cited, charged....I had a dismissed case before that I disclosed(DS160), should I also do the same with the I485?

q6: is a personal check ok for the payment?

q4: I am taking risk in sending in the vaccination record without transcription of a CS, will it be a problem ? You can include a copy of the DS3025 you were given at the Medical Exam.

q5: in filling out I485, do wee need to maintain what we filled out on the DS160 before? like for example other name used? or the maiden name if the "other name used" now since I am already married to my then fiancé? Include any names used before including the one before marrying

also,if I wrote an organization , should I do the same? Yes

lastly, on the question have u ever been arrested, cited, charged....I had a dismissed case before that I disclosed(DS160), should I also do the same with the I485? Answer all questions truthfully

20 May 2014: Wedding!

ROC

05/23/2017 -- Mailed ROC application

05/24/2017 -- NOA1 date

05/30/2017 -- NOA1 received in the mail

06/05/2017 -- Biometrics letter received

06/13/2017 -- Biometrics appointment

09/05/2018 -- Approved without interview!

09/13/2018 -- 10 Yr Green card Received
