Does my wife, who came in on a K1 Visa, need to get the I-693 medical examination for the I-485 AOS Packet?

Hello all,

We have an appointment in 3 1/2 hours with a Civil Surgeon in order to complete the I-693 part of the packet.

That being said, we just read in the I-485 instructions that since K1 Visa holders already had an overseas medical examination during the visa application process, than a vaccination report is all that is required.

Can anyone confirm this? If so, how do we prove we already had the examination? If the vaccination report is all that is required, is the vaccination record within the I-693 all that is required?

I just sent the vaccination part of the medical exam I took abroad. The K-1 packet the NBC has should already have my medical stuff in it. The medical examination place in London sent off the medical stuff to the embassy before they even issued an interview for me.

You need DS-3025 trancribed to I-693 by a civil surgeon if not all required shots were given, full medical examis not needed.

Hi,

If she has her DS-3025 filled properly (saying vaccination records complete or blanket waiver available), then she doesn't need to see the civil surgeon at all, all she needs is a copy of the DS-3025 to be included in her I-485 package.

Instructions are right, she doesn't need a new medical, as long as her medical overseas was done within one year of submitting I-485.

Print out this and bring it with you, especially page 5, part 3.

I don't know what proof of medical abroad the CS may ask for but they shouldn't ask for anything. It's not up to them to decide whether you need an exam or not, USCIS decides it.

Thank you all, this information reaffirmed what I was thinking.

Regarding the DS 3025 serving as the sole vaccination document, I have read that the top portion of the I-693 is still relevant. While the exam may not need to be taken, the beginning part of the first page still needs to be filled out and signed in the presence of the civil surgeon.

Again, confirmation?

No, you do not fill in the first page of the I-693 if you do not need to see a civil surgeon. Where did you read that you need to fill out the first part of the I-693 anyway? Follow the I-693 and I-485 instructions regarding medical exam/vaccinations for a K-1 applicant. You only use an I-693, properly filled out and signed by a civil surgeon if you need to see one. My wife never received a DS-3025 while applying for the K1 visa. That being said, I read we needed to fill out the first part of the I-693 and the vaccination page of that form from the link that Perdita D posted.

We were under the impression that the original DS-3025 was sent to the USCIS when she got her medical exam in Colombia. After that particular exam, when she asked about vaccinations they told her "don't worry, you'll get those in the United States."

I feel like we are missing something. Is it a serious problem if the fiance never receives the DS 3025 in the initial exam?

Hola!

This is Jeimmy and her husband Kevin. We are about to submit the I-485 AOS packet and we need some clarification on the medical examination/I-693/DS 3025 portion of the packet.

When I had my medical exam in Bogota I never received my DS 3025. In fact, the doctor did not look at my official Colombian vaccination records and informed me that I would be receiving my necessary vaccinations in the US.

Now I know that I do not need another medical exam however I am confused as to what I need to include in the packet regarding my vaccinations.

From what I have read, the DS 3025 is filled out during your initial exam and usually the beneficiary receives an extra copy. The official copy is put in the brown envelope and given to an official at the border when you enter. Because I never received the copy, my husband and I scheduled an appointment with a civil surgeon in order to get any of the four required vaccinations that were not included in my Colombian official vaccination record transcribed to the vaccination part of the I-693. According to the I-693 that he filled out we now have the required vaccinations.

Our question to anyone/Colombians out there is whether or not this is sufficient for the I-485 packet's acceptance. Did we need to fill out the DS 3025 instead? If so, how do we do that? When we showed him a copy he said he has never seen it before. If we do need it instead of a I-693, do we need to have my original official Colombian vaccination record transcribed to a DS 3025?

Are we actually totally fine and worrying for no reason?

You don't fill out the I-693, the civil surgeon does. I filled out part one by myself and the civil surgeon accepted the pre-filled out part one, I'm not sure if all of them would accept it though. I however signed the form in front of the civil surgeon, like the instructions state.

If your wife is positive a proper vaccination sheet was included in her original k1 file, then she doesn't need to see a civil surgeon at all and doesn't need I-693. If she's not 100% sure, you need to bring any proof of vaccination you have to the civil surgeon, which he will then transcribe onto the I-693 form. To me it seems the 'don't worry, you'll get them in the US' statement refers to her vaccines, not records, but I might be wrong, of course.

Did she get any shots at the time of her k1 medical? If yes, then the statement above might refer to the records and not vaccines.

Did she bring any vaccination proof to her k1 medical? The shots needed for AOS are MMR, Varicella, TDaP and Flu (the Flu shot would be required it it was the Flu season at the time of her k1 medical).

I didn't have any vaccination proof from my childhood when I went to my k1 medical. They didn't give me any shots, they just told me to take care of it once I got to the States. I got a Flu and TDaP shot and my doctor did some blood tests which proved I had MMR antibodies so I didn't need another MMR shot and it proved that I had Varicella as a kid, so I didn't need that shot either. So with the papers I got from my doctor (proving I got the two shots and didn't need the other two) I then went to a civil surgeon and he filled out the I-693 based on the proof I gave him.

Therefore, if the applicant believes he or she only needs a vaccination assessment, the civil surgeon may perform just the vaccination portion of the medical exam and complete pages 1 and 5 of Form I-693.

Based on reading your merged threads:

The doctor in Colombia did NOT look at your records of immunization. So they would not have recorded any useful information on the form DS-3025. They took the easy way out and told you to get that done in the US. That's why they didn't give you a copy because they didn't bother with it. Based on what you have said, a DS-3025 is not going to clear you on immunizations for AOS.

(IF they had recorded your shots on DS-3025, and you had ALL the required shots up to date, then it would have made it's way to your AOS file and possibly cleared you on immunizations. That's what some people in this thread are saying to you, BUT it does not sound like Colombia included a properly completed DS-3025 in your file. So forget the DS-3025. It's over. A K1 can get a visa without shots or a DS-3025. )

Now you are in the US ready to file for AOS. Your shots will now have to be signed off by a Civil Surgeon (because they weren't done in Colombia).The medical form in the US is I-693. The Civil Surgeon will look at your childhood records and advise you if you need more shots to be complete. One thing you will need is a flu shot because flu season does not end until March 31. You can get that anywhere and take the proof to the civil surgeon just like you will take your childhood shot record. He will put everything you bring onto one form (I-693).

You may fill out your name an address on the form and take it with you. BUT your signature has to be witnessed by the doctor. That is about identification. He has to see photo ID and know the person he examines is the person present and signing the form.

He does not have to know you had an exam before. That is explained in the Q&A document you have a link for. It is not his responsibility to verify your previous exam. If you lied and said you had, then USCIS deals with that and won't give you a greencard until they see an exam. USCIS has yours from Colombia and will use it. The Civil Surgeon does not have to see it. That part is between you and USCIS. He is only signing that he properly identified you, witnessed you signing, and filled out your immunizations.

So all you need from the civil surgeon is the vaccination page completed. The problem you will encounter is many civil surgeons don't deal with K1s who already had an exam. They think every immigrant needs a full medical exam, a TB skin test, and immunizations. The instructions that go with the I-693 say you do not need an exam, but many civil surgeons won't even listen to you. They say full exam or nothing. You will have to call around until you find one who seems to understand the K1 situation and will do your immunization page without an exam or TB test. $25-$50 might be the expected fee for that. They can charge anything they want but keep calling to find a reasonable doctor with a reasonable fee. I called a dozen once, in the Los Angeles area, to help somebody and they would not listen. I am well versed in the medical requirements and knew all the arguments, but they simply refused to budge off their full medical in the $240 range. Very frustrating. Finally one office manager listened and asked if I could fax her where those things are written because she had never heard of it before and wanted to learn. The others were very abrupt and rude. Thank you so much for everything this really narrowed it down. We did all of what you suggested yesterday (Got the I-693 first portion and vaccination page filled out minus the actual exam) so all's well that ends well.

I think our big hang up was that the DS 3025 has this laundry-list of vaccinations which we imagined were needed during the K1 application process. Since we never received a copy we were led to believe that in reality we needed far more vaccinations than the 4 being given by the civil surgeon.
