Can we extend K1 visa due to serious illness of visa holder???

My fiance and I submitted our petition in March 2014, it was approved by June 2014 and he interviewed and received his K1 visa on June 23, 2014. He stayed in his home country to work and save money and purchased his plane ticket for December 13, 2014. However he was just diagnosed with Hepatitis A and we are both very scared that he will be too sick to fly before his visa expires (the doctor ordered strict rest for one month, maybe more or maybe less depending on how severe the infection is). His visa expires December 23, 2014.

Does an extension of the K1 visa even exist? Please help us!!

From what I know, you can't extend it but you can have it re-issued and must have a real good reason (which I think you have). Call the embassy and ask them for more info. Do this fast and don't wait till the visa expires.

D-Day is December 20, if I calcuated it correctly. He does not have to be 100% in order to fly, and consider that if he gets here sooner instead of later, he can have access to superior US medical treatment (just make sure there is insurance to pay for it).

I have cancer and flew (a lot farther than Guatemala) while I was on high dose chemo. It was a horror, but I made it.

He has Hepatitis A though, and I think that's a highly contagious liver infection. I think the embassy will require another medical before re-issuing a visa.

How do you figure December 20? The expiration date says December 23, does he have to enter three days before expiration?

The problem here isn't really discomfort, but endangering his life due to liver swelling/ risk of liver rupture.

You will have to decide something with your physician opinion. If he is able to fly then consider that route. Looking for extension will have a negative impact since he is diagnosed with communicable disease. It will take considerable amount of time to do another medical and get cleared by US embassy.
