DS117 got denied. Reapplying for green card

My mom overstayed her stay outside the US. She applied for a returning visa but got denied. I'm reapplying for her but have some questions:

1. How do I apply? submit an I-130 for her?

2. Does she need to return her green card before I apply for her?

3. Item# 9 and 10 on I-130 form ask for social security and Alien numbers? Should I put her current social security and alien numbers or are these supposed to be left blank in her case?

4. Will she be getting a new alien number and should she apply for a new social security number when she comes back?

2. Not sure, embassy may ask her to return it when she goes for interview.

3. Put her current SSN and Alien number, SSN is one for life.

How long was she outside of the US that she had to apply for an SB1 visa? In any case if she lost her LPR status she has to start over from scratch so yes you would start with an I-130. Then the NVC process, affidavit of support, civil docs, medical, you know the drill.
