I-134 is a confusing form !!!! Help !

Hi everyone ,Perhaps I am going a little brain-dead from the paperwork !!!!! Does anyone know what to put at the end of form I-134 in front of "Amended" ? It wasn't on the sample form so I'm confused ,and also for the submit date of I-129 F should I put the date the USCIS get the file or the date of NOA 1 ? Thanks so much in advance .

I believe u put ur NOA1 date. That's what I did.

I don't see what place ur talking about that says "amended". Where is it? I see all the numbered questions and then the signature box.

Oh I see, it's at the end of the form, under the part "Oath or Affirmation of Sponsor" .Actually it's the paragraph before you sign the form .It has a fillable box

If you mouse over the data entry box for the 'Oath' section, it tells you what it is about and instructs you to enter the sponsor's full name.

So we put our name in there as petitioner? The example form doesn't show that. :/

It isn't on the example form for 2 reasons, one is because the example cannot include the mouseover info that is embedded in the original pdf file of the form you download from the USCIS site,and because I believe the example form was created before the USCIS added the instructions in the mouseover popup that finally explained what they wanted in that space.

It used to be left blank because it had no instructions. Then, it said to Enter relationship, which made no sense, so was also just left blank. Finally, they added this popup to let people know what they actually wanted to go there.

Yes, the sponsor filling out the form puts their full name there to indicate they acknowledge reading the liabilities in the instructions. See the attachment I am adding to show this popup info box.

Should I use any specific version of Adobe Reader ? Cause mine doesn't show the mouseover info !

Yes, the answer for #11 on the example form is correct. You could alternatively check the 'intend' box and still put 'K1 VISA PROCESS FOR PERMANENT RESIDENCE' on the line.

And not sure if you need to update your adobe reader to see the mouseover, or to download the form to your computer and open it with Adobe Reader. It shows up for me in my browser view just looking at the form at the USCIS site, and when I open the form in Adobe on my computer.

You are welcome. If your I-134 was already completed and sent to the foreign fiance(e), I would just leave it as is. I don't think leaving it blank still will have too much of an impact at all. If it hasn't been sent yet, then adding the name there is easy enough to do.
