Yes you do need a police report, you will need it when you go for the interview at the embassy. Any country where you lived form 6 months or more you will need it from. They told my fiance it would be 1 month to get it, of course every country is different.

As far as i know there is no form. You have to go to the police department closest to you and ask for one and they will give you their form. I might be wrong about this so please anyone do correct me if i am wrong.
