A question on G-325A AOS form

Hi everyone. A question about the attached G-325A form for AOS.

I am a bit confused on what to answer to the last question in this form wherein it's asking for the applicant's name. Above name is showing her married name but the last portion of this form, the applicant typed in her maiden name. Is that how it should be? I am filling out my AOS forms and I would want to use my new married name since I am now using my husband's last name.

May 13, 2014 Visa on hand

March 13, 2014 Medical exam - third day (went back to St. Lukes for Tdap vaccine)

Timeline

Use the married name on all your forms. Put the maiden where it asks for maiden or other names used. The VJ example of the G-325A has that small mistake in it. The maiden name should not be there. The married name should. It should match the first line of the form. Admin has been asked to fix it before, but they have not.
