Ontario Long Form/Certified Birth Certificate

Hi guys,

I just received my long form and certified birth certificate in the mail yesterday and it was not quite what I was expecting. It is a copy of the form that my parents signed when they registered my birth. It is signed and certified by the province of Ontario.

Was just wondering if anyone has had any issues with bringing this to the K-1 interview. All other long form and certified birth certificates I have seen look completely different and are not a certified copy of a form. Obviously it is different in every state and province but I guess I just wanted some reassurance from fellow Canadians.

That is it, I think it is actually a "record of live birth', which was registered when you were born. It has both your parents names on it right?

Yes the document has both my parents names and signatures and other information such as the hospital I was born and the doctor who delivered me.
