I have attached the points outlined in our cover letter for our AOS filing. Use them as a check list. All documents can be found by doing a google search. Our case will be a little different then yours as far as the documents. We had previous marriages to contend with and you may not, so ignore what is not relevant to your situation. We had no issues (RFE's) and was approved without an interview.

How about I-693? Because I had medical in the Philippines, do I need to fill out the form?

You have to have a medical exam (I-693) done by a Civil Surgeon. They will give it to you in a sealed envelope and you have to include that with the AOS packet

Umm, isn't the medical from the Philippines(and any other country, for that matter) good for a year?

I seem to remember that. So you wouldn't need another medical. Just transcribing the vaccination record if I remember correctly.

If you go by the USCIS instructions for the I-485, page 4 section 7c, all you need is a copy of the vaccination record (DS-3025), no civil surgeon needed.

Thanks Hank!

I highly suggest printing a copy of page 4 and highlighting section 7c then attach it with the DS-3025. Seems some at USCIS don't know the requirements for AOS very well.
