I-693, vaccination work sheet, ds-3025!

we will mail our AOS application package soon. However, when reviewing the online information/instructions it says that in certain cases, people who had medical exams in their home countries still *may* need to see a physician here. I already had my medical exam in my home country and I have a vaccination worksheet and a sealed CD. My question is should I send this work sheet with my package without form 693 or should I again see a physician who is certified and have him/her fill out the form.

Basically, do we have to send this 693 form?

what will I do with this worksheet that I have several copies of (that shows my vaccination records that I received before the visa interview at the consulate a few months back)?

on my vaccination worksheet it says "for ds 3025"--so what does this mean?

I am done with the AOS package except from this crazy vaccination form part.

one last question, do I need to send evidences of my relationship with my AOS package (photos, bank accounts)?

If the medical was <1yr ago, and the ds3025 was all filled out properly and says "vaccine history complete", send only a copy of the ds3025 - you don't have to attach the i-693.

No bonafide evidence needed at this stage, you'll have to take it to the interview in case you get one (about 50% people who adjust from K1s don't).

NB: some people who got all the required vaccinations and only sent the ds3025 for their AOS -including myself- have got an RFE for the i-693. Since this doesn't seem to happen often, I wouldn't spend more money to have a civil surgeon report your vaccination history on a i-693 for your first submission. I would only do it only if you get an RFE for it.

I have Vaccination documentation worksheet completed by panel physician before i came to USA. some people only sent this paper with their aos package (wo i-693 form) and they didnt have rfe . but everytime I call USCIS to figure this silly thing out they tell me to see a physician. ok. BUT they also tell me that i wont need to do that if I had my medical exam. within last one year. Theres no clear info even we talk with them. each of them tells sth different. Im confused.

I wouldn't necessarily trust whatever the USCIS call center says - they are well known to give misleading information. You're good to go with your copy of the Vaccination documentation worksheet (given it states "vaccine history complete"). Worst case scenario, you'll get a RFE for the i-693, pages 1 and 5 - at which point you'll see a civil surgeon to have them completed. That's exactly what I did. No big deal at all.

I just sent my package wo i-693. For some people only vaccination worksheet paper was enough to be done wo rfe. one of my friend will send her aos package tomorrow and she will send only worksheet wo i-693. on her worksheet it says vaccine history incomplete ( bc for some of the vaccines Age is not apPropriate ). we will wait and see. nothing makes 100% sense in this process. good luck to all

If they send an RFE(Request for Evidence) for an I-693 from a civil surgeon, then that is all you need to send them. You send what the RFE asks for. You do not start over with a whole new application for an RFE.
